Death Certificate Application

The clerk’s office only maintains records for deaths that occurred in The City of Wyandotte. In order to obtain a certified copy of a Death Record you must:

1. This document is available to view in PDF format. Completed and signed Death Certificate Application
2. Copy of driver’s license or sanctioned state identification.
3. Cash, check or money order payable to The City of Wyandotte.

The fee for a certified Death Certificate is $15.00 for the first copy, and $5.00 for each additional copy. Requests for additional copies must be made on the same date as the initial copy. When requests for additional copies at a later date are made, the initial $15.00 fee applies to the first copy requested.
